I have a 1UZFE in my 98 GS400 and went to pull #1 spark plug out and it seemed very hard to pull out, I pulled the next plug (#3?) and it seemed better, but didn't come out with the ease that iron head engine plugs come out. Engine was cold when I tried to pull them.

Any tips/advice? I don't want to break a plug off in the head.

These plugs have 140,000 miles (225k km) on them, so they need to be replaced, Lexus had them out at one point and replaced one plug only under warranty.
 
I always use a breaker bar, or torque wrench when pulling plugs. Will give you heaps more control, as to avoid cracking the ceramic, especially when old and brittle.

A short 3/8 drive ratchet is always a bit dodgy when the plug "gives" especially when tight.
I never had to, but spraying a bit of penetrating oil in first couldn't hurt if their really tight.
