High Pressure Vacuum

I have some problems with one of the vacuum outlets from the intake. The two outlets from the front of the intake has different size and one of them (smaller outlet) doesn't have any type of configuration to stop the vacuum line from coming loose. What I did was to pull them all out including the rear outlet. I drilled and tapped a 1/8" npt with 3/16" line outlet. At high boost, this the smaller outlet came off few times. See pictures below. The last few pictures are the rear outlet. Its drilled and tapped but no fitting yet.
