Do I need a early model flywheel or will 91-95 work for a 1UZ? (I am aware that the flywheel needs to be modified, and I am aware of some people's standpoint on that modification)

By the time you buy one, have it redrilled, drill it for your clutch and then turn or machine the ring gear you would be better off buying a new flywheel from NZ or Australia given the Dollar parity right now.
This would give you a 100% safe flywheel and you can have it set up for whatever clutch you want first go without any risk of having to pull it out again to fix some minor point that wasn't done quite right first go.
